It's a red temper. Everything is real, the post-processing in PS was mainly playing with a contrast, saturation and balance of colors etc.

If that's supposed to be blood on her face, the color---is a fail. It's too magenta. Blood is more red/orange than that. Great image though. It just peeves me to see "blood" that doesn't even look slightly plausible.
The red in the apple looks scary, I like that part very much.

When I first saw it in small view, at first glance, it looked to me like she was eating an egg. XD I thought the point was she was, like, eating life or something. Ha.
