Hmm weird, I'm having the same problem, except the bones that are getting left behind actually do have a parent. And this tutorial doesn't fix it. Actually, the parents for the bones being left behind are red in the PMX editor. Do you know what might be happening?

 
What's even more weird is that when I turn on physics the problem is fixed O.o any idea why this might be??

Good explaination but you should add a point, useful in a lot of animations: the "all parent bone" added by PMDE (and also PMX editor), is actually the "Motherbone". Which should be renamed in japanese in order to allow some motions recognizing it

Japanese name: 全ての親

Well, the procedure is correct, but the English PMD Editor don't works with PMX models and is not so easy to spot that option on the Japanese interface (the menu has more entries). There is an English patched PMX editor around there, but I have not been able to make it work.
