I think the intestines and other internal organs are kept largely intact, they can get pushed into a much smaller space. Waste simply gets collected in bags through stoma in the abdomen. "Functioning" is ok, but life expectancy isn't spectacular following a surgery like this (partly due to complications, partly because of whatever disease/accident/infection caused the need for the surgery to begin with).
