Description I've been restarting a RuneQuest campaign from years ago with my friends for a few weeks now.
I made this sheet in order to record our coming battles blow-by-blow.

The sheet can work with any generic RPG, but I've made this with RuneQuest 3rd Edition combat rules in mind.

The sheet is subject to revision as I get practical experience with it.

First revision 15.09.2012:
Resized the cells: Cells take less total space in order to allow space for a hole punch on either side of the sheet and a further two cells have been squeezed in.
Additional info: attack and defense have been separated into their own boxes with appropriate checkboxes; damage roll has its own field; removed weapon type space; added defensive action box to point out successful or inadequate defensive attempts; location line has space for location AP; location status split to boxes for attack effect on target and hit location, giving info whether an attack causes a knockback, a knockdown, a knockout, bleeding of a location, crippling of a location or the maiming or severing (combined to destroying) a location.
Notes section decreased.
Changed info names: Weapon renamed to Attack; Miss/Parry checkbox split to Dodge and Parry checkboxes; Maimed/Severed location marker renamed to Destroyed.

Notes of use:
Boxes named accordingly, check whether attacker is a Player, a friendly NPC or a hostile NPC.
If an attack is rolled successfully, check the appropriate boxes, otherwise leave empty; in case of fumble check only the Critical attack box.
If defender succeeds in defending, check appropriate boxes (dodge and critical parry negate attack, while normal or special parry lessens damage).
Fill appropriate line with total damage (rolled+bonus+damage modifier);
Note hit location and total AP defending said location (in case of parry add weapon/shield AP).
Target status effects are noted in appropriate checkboxes in cases of knocking an enemy back due to special attack or size; knocking the enemy down due to limb crippling; or knocking the enemy out due to decreased HP or crippled head.
Location status status effects are noted in appropriate checkboxes in cases of causing crippling damage to an enemy limb/location (reducing location to 0 HP or less); causing said location to bleed (reducing location to 0 HP or less); or destroying (severing or maiming location at -N x HP) a location.
Notes are used for free-form explanations or extra info, e.g. whether an enemy had fatigue effects or bled to unconsciousness or death outside combat action.
