A completed JPEG version for Paint users. You can freely alter the canvas size and "Copy & Paste" to increase or decrease your chart.
By fuumika 's request, my simpler version is available for use as well. If for any reason you don't wish to use Fuu's, feel free to use this one.
Heart outlines are the same colour as the labels given above, so you can use the Dropper tool to take that colour and fill in any heart that applies to that character. Remember that you should only fill the heart that best describes your OC's relation with the other. Do not fill from "Contempt" and then fill all the way to whichever one applies.
Be sure that you also describe the relationship in the description so that we know how this friendship conclusion came to.
The "In A Relationship" tab is special, because your OC can either like or dislike someone greatly, and still have them in a relationship. So it tells us "Is the relationship forced, or is this their decision?". Something like that.
The blanks on the left can either be used for names or character faces. Maybe both to be creative? Either way, you need it so we know who the relationship info applies to. The top-most can be your OC's face or name.
