Mario does care about him, there’s just other seemingly more important things he’s worrying about right now. But you’re right, it doesn’t mean he can’t pay attention when his brother is saying he’s sick. But Luigi doesn’t like being told off, so I doubt if he’ll bring it up for a second time after this.

Lol no don't worry, he's not drunk. He's out on an adventure with his brother, and he got poisoned by an enemy they were fighting. He tried to ignore it, but it didn't wear off as he assumed it would, it got worse. But he didn't realise it was the enemy that poisoned him, he didn't know why he felt sick. So when he tried to explain it to Mario he didn't have a good reason backing up how he was feeling, and so Mario didn't believe he was really sick.
